Freestyle Libre Users:

If you have an account on the FreeStyle Libre 3 app, glucose data is automatically uploaded to LibreView. You can then share data with NextGen Diabetes Care by:

1. Opening the app.

2. Clicking the menu (three stacked lines) at the top left corner.

3. Clicking "Connected Apps".

4. Clicking "Connect" next to LibreView.

5. Enter our “Practice ID" which is 6036913771.

Dexcom Users:

If you have an account on the Dexcom G7 App glucose data is automatically uploaded to Dexcom Clarity. You can then share data with NextGen Diabetes Care by:

1. Opening your Dexcom sensor app on your smartphone.

2. Tap the Connections Tab.

3. Tap Clarity Clinic and tap “Continue”.

4. Enter the Clarity Clinic Code 6036913771

5. Confirm clinic details and tap “Done” when finished.

6. Clarity clinic will remain an active connection in the Connections Tab of your Dexcom CGM App.
